In this online e-logbook, we upload our patients de-identified health data shared after taking his/guardian's signed informed consent. Here we discuss our individual patient problems through series of inputs from an available global online community of experts with an aim to solve those patients clinical problems with collective current best evidence-based inputs. Date of examination:-21/09/2021 A 35yr old female presented to OPD with chief complaint of increase in bloodpressure since 3 days. History of present illness:- Patient was apparently asymptomatic 10 days ago then she developed fever with chills, cough and cold, which subsided after taking medication for 3 days. On monitoring her BP it was 180/100mm of Hg.she was advised to take tab TELMA 20mg PO/OD and was asked for regular BP monitoring at home. On 18/9/21 patient came for follow up i/v/o high BP 210/140mm of Hg. HISTORY OF PAST ILLNESS:- No evidence of target organ damage. No history of blurri
