Date of examination:-21/09/2021

            A 35yr old female presented to OPD with chief complaint of increase in bloodpressure since 3 days.

History of present illness:-

           Patient was apparently asymptomatic  10 days ago then she developed fever with chills, cough and cold, which subsided after taking medication for 3 days. On monitoring her BP it was 180/100mm of Hg.she was advised to take tab TELMA 20mg PO/OD and was asked for regular BP monitoring at home.

On 18/9/21 patient came for follow up i/v/o high BP 210/140mm of Hg.

HISTORY OF PAST ILLNESS:- 

No evidence of target organ damage.

No history of blurring of vision,Angina,reduced urineoutput.

History of gestational hypertension 10years back.

She had 2 pregnancies.

1st pregnancy-8th month- high BP-put on medication -normal delivery was done.

2nd pregnancy-8th month-high BP-put on medication Csection was done.

No history of Diabetes mellitus,TB,Epilepsy, thyroid.

Family history:-

Patient's father has HTN since 1 month.

Treatment history:-

No past treatment history for HTN.

PERSONAL HISTORY:-

Diet:-Mixed

Appetite:-Normal

Sleep:- Adequate sleep.

Bowel and bladder:- normal

Addictions:-No

Menarche:-12yrs

Normal menstrual cycle with normal flow

She is married and has2 children.

GENERAL EXAMINATION:-

Patient is conscious,coherent and cooperative well oriented to time place and person.

No bilateral pedal edema.

No H/O Cyanosis

 No H/O  Clubbing

No H/O Lymphadenopathy.

VITALS:-

TEMPERATURE:-afebrile

PULSERATE:-80bpm

BLOOD PRESSURE:-170/110mm of hg.

Respiratory rate:-20cycles per min.

SpO2:-98%

HEMOGLOBIN:-10.7gm/dl

Total count:-36000cells/cumm

Lymphocytes:-45%
